My classmates came to Kuching for 5 days and Diana and I actually hosted them by bringing them around the town. Basically, it was a "Foodthalon". Last night's dinner was the most memorable one as we took a sampan across the river to eat ayam penyet (Originally, we wanted to eat ayam pansuh but when we reached there, they told us that we were supposed to book in advance) at My Village in Kampung Boyan. The place was very traditional with very nice atmosphere if you exclude the wild cats, slow service and free music from the mosque. After dinner, we went to Open Air Market to have supper. <br />

We managed to pick up different types of heart murmurs. =) During our first ward round, the matron scolded us for washing hands together in one sink. She said that was the first time she saw people washing hands that way. Then, during our second ward round (this morning), the matron scolded us again for not wearing our name tags. There are a few investigations which are most available and frequently used tests to identify coronary ischemia in clinical practice.<br />
<br />
Exercise testing — Exercise testing appears to be the most suitable laboratory diagnostic test to document coronary ischemia in asymptomatic individuals (eg, patients with no history of CHD) and in those with a history of CHD or exertional angina. Exercise testing is frequently used to screen high risk, asymptomatic persons to identify those with asymptomatic CHD. Conventional ST segment analysis during ETT is moderately sensitive in detecting CHD. As a result, the diagnosis of myocardial ischemia by ETT in asymptomatic individuals must be confirmed by radionuclide imaging techniques (eg, thallium perfusion scintigraphy or exercise ventriculography) before the subject is labeled as having coronary ischemia.<br />
<br />
Holter monitoring — Holter monitoring is the second most frequently used diagnostic test for coronary ischemia. It has the advantage of providing long-term ECG recording of ischemic and arrhythmic events while patients are engaged in routine daily activities out of the hospital. Episodes of transient ischemia during Holter monitoring are diagnosed by a sequence of ECG changes that include a flat or downsloping ST depression of at least 1 mm, with a gradual onset and offset that lasts for at least one minute. <br />
<br />
Nuclear and echocardiographic imaging studies — Tests other than routine ETT and ambulatory monitoring may be necessary in certain circumstances. As an example, nuclear imaging tests such as stress thallium scintigraphy or exercise radionuclide ventriculography are recommended for the evaluation of coronary ischemia in patients who have an abnormal baseline ECG (eg, left ventricular hypertrophy or strain, bundle branch block, preexcitation syndrome), and those receiving digitalis, phenothiazines, or other drugs that produce repolarization changes. Pharmacologic stress tests with dipyridamole or adenosine plus thallium scintigraphy, or dobutamine stress echocardiography, can be utilized in those patients who are unable to ambulate or exercise (eg, due to advanced peripheral vascular disease) <br />
<br />
Electron beam computed tomography — The severity of coronary artery calcification (CAC) on electron beam computed tomography (EBCT), as determined by a calcium score, can identify asymptomatic patients at high risk for coronary heart disease. However, it is not certain if this translates into identification of asymptomatic patients who have coronary ischemia. <br />

My shoulders suddenly felt very light. The first week after my EOS 5, I was busy finding a house to stay in Seremban. I finally found one in Seremban 3 and I will be staying with my other 3 friends. The house is rm800 per month but it is unfurnished. Anyway, after settling the accomodation, I was busy packing and moving all my things to the new house. Man, moving house is definitely very tiring. <br />
<br />
I took a short break with my family and relatives after that. It was a great reunion indeed. =) <br />
<br />
After the short break, I have been slacking around in Kuching till today. Haha Well, at least I go for hospital attachment at SIMC and I get to learn new things every morning. I can say that is the most beneficial thing I have done so far during my holidays. In the afternoon, I will be baking cookies and cakes with my cousin and his wife while being a babysitter as well. Haha <br />

I watched movies in the cinema very frequent till there is no more movie for me to watch right now and I have to wait for new movies to be out. <br />

When patient comes to you, first, identify the age. If patient is old, it is more likely to be aortic stenosis. If patient is young, it can be mitral regurgitation, aortic regurgitation, mitral stenosis or congenital defects such as ventricular septal defect, atrial septal defect and patent ductus arteriosus. Second, check for clubbing and scars. If none is present, congenital defects are unlikely to occur. Next, palpate or auscultate for the apex beat. If it is displaced, it is either mitral regurgitation or aortic regurgitation and hence, mitral stenosis can be ruled out. Now, one is left with either mitral regurgitation or aortic regurgitation. Auscultate the back for murmur. I am glad I passed and it is over now. Thanks to my helpful friends, lecturers and not to forget, my mentor from Kuching. =D I will miss my friends who will be going to overseas instead of Seremban to do their clinical years especially Sin Dee and Hui Hwen. Not to forget Jonathan as well who was so willing to be my patient for me to practise my clinical skills. <br />

We were divided into many groups to carry out our Community Medicine project. My group was assigned to Nilai. There were 36 of us. We further divided our tasks among ourselves. Some of us studied the organizational structure in the Klinik Kesihatan Nilai. Others did the household survey and the research project. I was involved in household survey. For the household survey, we did a survey to know how much the people in Nilai know about Diabetes. Our instrumentation was a questionaire. Wow, I thought questionaire was an easy job. It was not at all. We walked so much in the hot sun around the big neighbourhood in Nilai and it was from house to house. There were dogs in some houses and we were rejected by the people in some houses. We had to collect at least 250 data and we managed to collect more than 300 data. For our research project, we did a cross sectional study on how much people in Nilai know about Dengue. Tomorrow, we have to start to key in all the data and use the SPSS software for our data analysis and results interpretation. Next week, we have to present it to all the doctors who specialized in Community Medicine. <br />

We both reached Penang on a late Friday night. Wen How brought us back to his apartment and we stayed in his room for 2 nights. <br />

The next morning, he gave us his car and GPS to go out because he had to work and he could not bring us out. We drove the car WITHOUT any LICENSE. Sze Ying and I started our day off with dim sum at Tian Tian Lai for breakfast. <br />

We wanted to go to P Ramlee's house after that but our GPS ran out of battery. That was when the REAL adventure began for the both of us. Two lost Kuching drivers in Penang. We decided to go to Gurney Plaza instead to find a handphone shop to charge the GPS. It took us an hour to find the directions to Gurney Plaza. We asked so many people, from going into a church to going into a hospital to ask for directions. We drove along Jalan Pangkor for more than 5 times. When we finally reached Gurney Plaza, we were so relieved. The shop charged us rm10 to charge our GPS there. We went shopping while waiting for it to be fully charged. <br />

At first, it was rather balanced but right now, it seems like my playing time is more than my studying time. All the silly moments. haha We tried playing Zero point again but since we are older now, our stamina decreased. When we jumped for too long, we would feel dizzy as well. I guess the game is no longer suitable for us anymore. We prefer to sit down and talk instead now. We developed a new hobby. We go for karaoke now. That doesn't mean that we can sing. Usually, we lose our voice after singing and screaming in the room. We eat more since we can drive around now. We walk less compared to last time. Since I was so free back then, I agreed with her. I went for classical guitar lessons with her for almost 2 years. We took exams together as well. Due to PMR, we both stopped learning to play a classical guitar. Last year, one of my coursemates, Sim, found out that I used to play a classical guitar and he asked me to teach him despite the fact that I told him I had forgotten on how to play a classical guitar. Since I was busy preparing for my End of System 3, I did not invite him to come to my place till few weeks ago at night. He told me he could not play (what a lie). When I played each note on the guitar and I asked him to tell me which note it was, he could answer all of them correctly. I started to doubt him for not being able to play a classical guitar at all. Yes, it was indeed a lie. haha He could play so well and he played it so much better than I could play. In the end, he entertained Nicole and I with all the romantic songs. Haha Romantic songs such as Right here waiting , Now and forever and Romance Te Amor. After that, we had free taekwando lessons from him. He looked gentle but he is a black belt master. He taught us self defence as he said girls must know how to defend themselves from danger. <br />

Back in high school, first day of school was a total holiday. I just had to register myself and buy the new books. Now, first day of school means first day of stress. Why? On the first day of school, I had problem based learning (PBL) at 9am to jot down all the learning issues which I have to do research on and present them this Friday. The topic was on Diabetes Insipidus. Then, I had clinical skills in the afternoon. We learned how to inspect, palpate, percuss and auscultate the thyroid gland. After that, I had to go for lectures in the evening till 630pm. I also have to take Malaysian studies for the whole semester. It is time to make notes for every lecture and memorise my notes everyday. I am doing Endocrine system now. It will be followed by Reproductive system and Renal system.<br />

I went out with Grace. We knew each other since primary one. I sat next to her in school for many years. Back then, she called me Ugly Teo and yeah, Ugly Teo remains as one of my nicknames now. We had so many sleepover parties together during high school. Not to forget our trip together to Sarikei after form three. Wow, we spent so many hours in the bus. Haha We had fun dressing up as twins in Sarikei. After all, no one knew us there. Yeah, we embarrassed ourselves there by wearing bandanas in town. We used to go to the arcade very often to play the games there. Recently, we met up to do what we used to do when we were younger. Other than the arcade, we went to sing at Enter K.<br />

Li Hwa had a long break. Twice my break. I spent most of my time with her. My companion when I feel bored. Haha We knew each other since primary four. She was well known to be late for school. =P We used to play dance dance revolution together after school. We car pooled to go to tuitions. One funny memory about her. A car ran over one of her slippers and the strap broke when we went clubbing at Soho during New Year Eve back in high school. During my two months break here, we went out to exercise. We went to try line dance, brisk walking in Stutong Park, basketball in St Faith Church and badminton in Stapok. We went window shopping. One thing I will always remember, we went for eyebrow trimming together. Why? It was the first time for the both of us. We bought local anaesthetic spray to spray our eyebrows before going for it. Haha It was damn hillarious as the pharmacist laughed at us and even told us that local anaesthetic spray should be used for eyebrow tattooing. Not eyebrow trimming.<br />

Fiona. My one and only friend who is a musician. haha The first time I met her was in primary one. We used to compete with each other in terms of height. Haha My partner in crime back in primary school. We used to turn the clock in Chinese class to make it end faster and the teacher only realized it after two weeks. Digimon was our favourite game. She left for New Zealand after form three but we are still as close as ever up till now. She is back for good now to work in Kuching. Recently, we met up for food outings almost every day. Coffee and cakes. We will usually play badminton together once a week in Stapok.<br />

Before starting my first day there, I thought I would be able to see face lift, mole removal, pimple removal, botox treatment and collagen injections. After spending one week there, I realized what I thought I would see only exist in a private hospital. In a government hospital, I saw patients with cleft palate going for palatoplasty, cleft lip with congenital hydrocephalus, spina bifida occulta and hypoplastic left lung going for lip repair, curbuncle at left nasolabial and upper lip going for saucerization and left temporal basal cell carcinoma going for wide local excision and full thickness skin graft. Other than that, I saw patients with different degrees of burns. I learned how to manage a patient with burns and how to do different types of dressings from Sister Paat. On every Monday and Wednesday, there will be operations in the operation theatre. Diana and I followed the plastic surgeon into the operation theatre to watch the operations. Basically, a lip repair takes about an hour and a palatoplasty takes about two hours. There will be follow ups in the clinic every Thursday and Friday.<br />

Other than that, there is a room for resuscitation, a room for X ray, two rooms for dressings and a room for operation. There are two teams for ambulance call. When the buzzer rings three times, first team will go out and when the buzzer rings two times, the second team will go out. If the buzzer rings once only, it means a patient has been transferred from yellow zone to red zone. There are two Accident and Trauma physicians in the department. Every day, there are many different cases. Road traffic accident is one of the most common cases every day there. Every Wednesday and Thursday, the doctors will have discussions among themselves. During my week there, they had a discussion on non traumatic acute abdomen. The diagnosis includes history, physical examination and imaging such as computed tomography scan, abdominal X ray and ultrasound. As for the management, it includes resuscitation (Airway, Breathing and Circulation), surgical intervention and close monitoring. Sometimes, my friends and I would follow the patients to be scanned. <br />
